You will utilize state-of-the-art electronic medical record (EMR) systems, including Canopy and Greenway, and have access to on-site labs and specialist consultation platforms. Our care model emphasizes patient-centered visits, with manageable daily volumes to ensure each patient receives thorough and individualized care. The Nurse Practitioner (NP) will also participate in community programs and activities supporting holistic patient wellness.

Nurse Practitioner (NP) - Summary
- Full-time Nurse Practitioner (NP) role within a value-based healthcare model

Duties & Responsibilities
- Provide comprehensive primary care services to Medicare patients
- Conduct patient visits, including regular, diagnostic, and new patient assessments
- Manage a patient volume of approximately 13-16 patients per day
- Coordinate with a multidisciplinary care team, including physicians, scribes, medical assistants, and social workers
- Utilize EMR systems (Canopy and Greenway) for accurate documentation and patient management
- Participate in preventive care initiatives to reduce hospital admissions and improve outcomes
- Engage in ongoing patient activities in the community room, such as exercise and wellness events
- Access on-site laboratory services and specialist consultations via RubiconMD
- Contribute to a positive patient experience and high-quality care delivery

Qualifications & Requirements
- Graduate of a Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program
- National certification as a Nurse Practitioner (FNP, AGPCNP, ANP, or GNP)
- Active, non-probationary state Nurse Practitioner (NP) license
- Active DEA license
- Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs) preferred
