Nurse Practitioner (NP) - Summary
- Deliver equitable, value-based healthcare to a diverse Medicare patient population
- Work within a collaborative care team of Nurse Practitioners, Primary Care Physicians, and support staff
- Enjoy a patient-focused environment with resources designed to optimize patient care and outcomes
- Utilize state-of-the-art EMR systems (Canopy and Greenway) and on-site lab services
- Engage in a workplace that prioritizes preventive care, patient education, and community engagement
Duties & Responsibilities
- Conduct comprehensive patient assessments, including diagnostic, preventive, and follow-up visits
- Manage an average daily patient volume of 13-16 individuals, providing care every three months or more frequently for complex cases
- Coordinate patient care with a multidisciplinary team, including medical assistants, scribes, and social workers
- Document care in EMR systems, ensuring accuracy in HCC, ICD-10 coding, and health risk assessments
- Participate in community room activities to encourage patient engagement and wellness, such as exercise, events, yoga, and wellness days
- Promote preventive medicine, chronic disease management, and health education
- Utilize on-site laboratory and access to specialist consultations through RubiconMD
- Support patient access to care, including coordination of transportation within 5 miles of the clinic
Salary & Benefits
- Competitive base salary ranging from $120,000 to $140,000, with additional performance-based bonuses

Qualifications & Requirements
- Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of a Nurse Practitioner Certificate Program
- National certification as a Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
- Active, non-probationary Nurse Practitioner license
- Active DEA license
- Experience or proficiency in HCC documentation, ICD-10 coding, and health risk assessments preferred
- Strong commitment to delivering patient-centered, value-based care
