We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our team and deliver high-quality, value-based healthcare services to a diverse Medicare patient population. As a Nurse Practitioner (NP), you will work in a collaborative care environment that prioritizes patient outcomes over volume, supported by a multidisciplinary team including scribes, medical assistants, and social workers. Our model is designed to reduce hospital admissions, promote preventive medicine, and create a positive patient experience, all while offering competitive compensation and rewarding performance-based outcomes.
As a Nurse Practitioner (NP), you will have the opportunity to make a significant difference in patients' lives by providing comprehensive care and leveraging the resources of a well-equipped clinic. The position offers a structured work schedule, manageable patient volume, and access to the latest electronic medical record systems (EMR) such as Canopy and Greenway. You will also benefit from on-site laboratory services and easy access to specialist consultations through RubiconMD.
Our clinics foster a strong sense of community, offering transportation for patients within a five-mile radius and providing a variety of patient activities in our community room, such as exercise classes, wellness events, yoga, dance, and spa days. As a valued member of our team, you will join a supportive environment with 3-5 providers per clinic, including a mix of Nurse Practitioners (NPs) and Primary Care Physicians.
Nurse Practitioner (NP) - Summary
- Deliver value-based, patient-centered care as a Nurse Practitioner (NP).
- Work within a multidisciplinary team to improve patient outcomes and satisfaction.

Duties & Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans.
- Provide preventive and diagnostic care to Medicare populations, aiming to reduce hospital admissions.
- Manage a daily patient volume of approximately 13-16 patients, with 20-minute regular visits and 40-minute diagnostic or new patient appointments.
- Coordinate care with a team of scribes, medical assistants, and social workers for optimal patient support.
- Utilize EMR systems (Canopy, Greenway) for efficient and accurate documentation.
- Collaborate with other healthcare professionals, including access to specialists via RubiconMD.
- Participate in regular follow-up visits with patients to ensure ongoing care and management of complex cases.
- Engage patients in community room activities to promote wellness and preventive health.

Qualifications & Requirements
- Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner (NP) certificate program.
- National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
- Active, non-probationary state Nurse Practitioner license.
- Active DEA license required.
- Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments (preferred).
- Bilingual Spanish-speaking candidates are encouraged to apply.
