Nurse Practitioner (NP) – Summary
- Provide primary and preventive care to Medicare patients in a value-based care setting.
- Collaborate with a multidisciplinary team—including physicians, scribes, medical assistants, and social workers—to deliver patient-centered care.
- Utilize advanced EMR systems (Canopy and Greenway) and on-site resources to streamline patient management.
- Enhance patient experience through regular follow-up visits, preventive care, and community engagement activities.
Duties & Responsibilities
- Conduct comprehensive patient assessments, diagnostic visits, and follow-up care in accordance with value-based care principles.
- Manage a daily patient volume of 13-16, with 20-minute standard appointments and 40-minute diagnostic/new patient visits.
- Coordinate care with the support of a dedicated care team, including scribe, medical assistant, and social worker.
- Document all encounters accurately using EMR systems.
- Engage patients in health education, preventive care, and chronic disease management.
- Participate in community room patient activities and support additional patient services, such as transportation within five miles of the clinic.
- Collaborate with 3-5 providers per clinic, ensuring a dynamic and supportive clinical environment.
- Utilize on-site laboratories and specialist access (RubiconMD) to facilitate comprehensive care.
- Ensure compliance with all regulatory requirements and organizational policies.

Qualifications & Requirements
- Graduate of a Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner Certificate Program.
- National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
- Active, non-probationary Nurse Practitioner license in good standing.
- Current DEA licensure.
- Proficiency in HCC documentation, ICD-10 coding, and health risk assessments preferred.
- Bilingual Spanish-speaking ability is highly valued for patient communication.
