Nurse Practitioner (NP) - Summary- Full-time Nurse Practitioner (NP) position focused on value-based care for Medicare patients
- Collaborative, team-based environment with strong support resources

- Provide comprehensive primary care to a panel of Medicare patients
- Conduct regular and diagnostic patient visits (average 13-16 patients per day)
- Participate in preventive care initiatives and chronic disease management
- Collaborate with a multidisciplinary care team including scribes, medical assistants, and social workers
- Utilize on-site laboratory services and electronic medical record systems (Canopy and Greenway)
- Engage with community resources including transportation and wellness activities for patients
- Maintain accurate documentation and adhere to value-based care protocols
- Work within a structured weekday schedule (Monday through Friday, 8am-5pm)
- Competitive base salary range with additional incentive bonuses based on outcomes

- National certification as a Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
- Bilingual Spanish-speaking candidates encouraged to apply
- Experience with HCC documentation, ICD-10 coding, and Health Risk Assessments preferred
- Demonstrates a commitment to patient-centered, value-based care
- Strong communication and interpersonal skills
- Ability to work collaboratively within a team-based model
- Detail-oriented with excellent documentation abilities
- Passionate about preventive medicine and managing complex patient needs
- Fluent in Spanish or experience in multicultural settings is a plus

- Transportation services available for patients within a five-mile radius

- Average clinic size includes 3-5 providers, a mix of Nurse Practitioners (NP) and Physicians
