The Nurse Practitioner (NP) will work collaboratively within a care team, including scribes, medical assistants, and social workers, to ensure equitable access to healthcare and improved patient experiences. As a Nurse Practitioner (NP), you will manage a panel of Medicare patients, averaging 13-16 visits per day, with a focus on comprehensive assessments every three months or more frequently for complex cases. You will conduct regular and diagnostic visits, collaborating closely with other healthcare professionals to coordinate care. The clinic is equipped with labs, phlebotomy services, and access to medical specialists via RubiconMD, ensuring patients receive timely and specialized care when needed. Patient transportation is provided within a five-mile radius, and the clinic hosts community activities to support holistic health and wellness.

- Active, non-probationary state NP license
- Active DEA license
