Nurse Practitioner (NP) - Summary
- Deliver comprehensive primary care services to Medicare patients in a value-based care setting.
- Function as part of a multidisciplinary team to ensure coordinated and effective patient care.
- Utilize evidence-based practices to improve patient outcomes and satisfaction.
- Employ advanced clinical skills in assessment, diagnosis, treatment, and preventive care.
- Leverage electronic medical records (EMR) systems such as Canopy and Greenway for documentation and workflow efficiency.

Duties & Responsibilities
- Conduct patient visits, averaging 13-16 patients per day with visit lengths tailored to patient needs (20 minutes for regular visits, 40 minutes for new or full diagnostic visits).
- Perform comprehensive health assessments, including health risk assessments (HRAs), and manage both acute and chronic conditions.
- Collaborate closely with other healthcare professionals, including physicians, medical assistants, social workers, and specialists, to deliver coordinated care.
- Document patient encounters accurately using EMR systems and ensure compliance with ICD-10 coding and HCC documentation standards.
- Participate in team meetings, case management discussions, and community health initiatives such as patient wellness activities, education, and preventive screenings.
- Support patient engagement through community room activities, including exercise, yoga, and wellness events.
- Utilize on-site resources such as laboratory services and phlebotomy, and facilitate patient transportation when needed.
- Ensure continuity of care by scheduling regular follow-up visits, typically every three months, or more frequently for complex cases.

Qualifications & Requirements
- Graduation from an accredited Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner Certificate Program.
- Current national certification in one or more of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
- Active, unrestricted Nurse Practitioner license in the state of practice.
- Valid DEA license for prescribing medications.
- Proficiency in HCC documentation, ICD-10 coding, and health risk assessments preferred.
- Strong communication, clinical judgment, and organizational skills.
