Nurse Practitioner (NP) - Summary- Full-time Nurse Practitioner (NP) role in Chicago, IL
- Focus on Medicare population and value-based care
- Collaborative, multi-disciplinary team environment
- Emphasis on patient outcomes and preventive medicine
- Conduct comprehensive health assessments and routine follow-up visits for Medicare patients
- Manage patient panels of 13-16 patients per day, allowing for 20-minute standard visits and 40-minute diagnostic/new patient visits
- Coordinate care with onsite lab services, scribes, medical assistants, and social workers
- Utilize EMR systems (Canopy and Greenway) for accurate documentation and coding
- Engage in proactive health risk assessments and preventive care strategies
- Participate in regular care team meetings and quality improvement initiatives
- Support community engagement through patient wellness activities and events
- Ensure safe, equitable, and effective care delivery in accordance with value-based care principles

- National certification as a Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- Bilingual Spanish speaking preferred
- Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments preferred
- Experienced in providing primary care to Medicare populations
- Skilled in EMR usage and evidence-based practice
- Able to work collaboratively in a multidisciplinary setting
- Strong communication skills, with the ability to engage patients and family members
- Dedicated to preventive medicine and continuous quality improvement
- Adaptable, organized, and committed to delivering equitable care
