Nurse Practitioner (NP) – Primary Care Summary
- Provide primary care services to a panel of senior patients, focusing on value-based, preventative, and holistic health approaches.
- Work as part of a multidisciplinary team, including physicians, medical assistants, schedulers, and referral specialists.

Duties & Responsibilities
- Deliver comprehensive outpatient primary care to a panel of approximately 450 senior patients, averaging 18 patient visits per day.
- Conduct thorough assessments, develop personalized care plans, and manage 3-5 chronic diseases per patient.
- Collaborate with a dedicated support team, including your own medical assistant, scheduler, and referral specialist, to ensure seamless patient care.
- Utilize value-based care principles to optimize health outcomes, reduce hospitalizations, and enhance patient satisfaction.
- Participate in regular administrative activities, with time allocated within your schedule for documentation and care coordination.
- Engage in interdisciplinary consultations with in-house specialists in cardiology, podiatry, and other relevant fields.
- Promote holistic health services including acupuncture, Tai Chi, yoga, and nutrition counseling for patients as appropriate.
- Ensure effective communication and education for patients and their families, particularly with bilingual (Spanish-speaking) seniors.

Qualifications & Requirements
- Current board certification as a Nurse Practitioner (NP) in Family Medicine or Geriatric Medicine.
- Active and unrestricted NP license for practice.

- Excellent communication skills and ability to work collaboratively within a multidisciplinary team.
- Bilingual proficiency in Spanish required to effectively serve the patient population.
