The Nurse Practitioner (NP) role will enable you to build meaningful relationships with patients by maintaining a reduced patient panel, ensuring you have adequate time to address each individual's needs. You will collaborate closely with a dedicated care and support team, including medical assistants and schedulers, to deliver comprehensive, patient-centered care. The clinic is designed to support both patients and providers, offering amenities such as door-to-door transportation for patients and administrative time built into the schedule for providers. The role is structured for optimal work/life balance, providing a Monday through Friday schedule with no night or weekend shifts required.
As a Nurse Practitioner (NP) in this primary care setting, you will focus on value-based care, managing a panel of up to 450 patients and seeing 18-22 patients per day. Your patient population will primarily consist of seniors managing three to five chronic diseases. You will have the autonomy to practice medicine the way it should be practiced, with the support of a robust care team and access to comprehensive resources designed to enhance patient outcomes.
