Nurse Practitioner (NP) - Summary

Duties & Responsibilities
- Conduct thorough patient assessments, physical exams, and diagnostic evaluations
- Manage chronic conditions such as diabetes, COPD, CHF, dementia, and debility
- Develop and implement individualized care plans aligned with each patient’s health goals
- Educate patients and families on disease progression, medications, nutrition, and preventive measures
- Coordinate care and collaborate with medical assistants, schedulers, referral specialists, social workers, pharmacists, and specialty providers
- Prescribe medications within the scope of practice and state regulations
- Deliver care in a clinic setting, with occasional home visits and telehealth as required
- Maintain accurate and detailed documentation in the electronic medical record (EMR) system, with Athena EMR experience preferred
- Follow value-based care protocols and risk-adjustment documentation standards
- Participate in quality improvement initiatives and interdisciplinary team meetings

Qualifications & Requirements
- Master’s degree in Nursing
- Board-certified Nurse Practitioner (NP) in Adult, Adult-Gero, or Family disciplines
- Minimum 2 years of outpatient primary care experience required
- Experience with value-based care strongly preferred
- Ability to practice autonomously
- No new graduates; experienced NPs only
- Active state-specific Nurse Practitioner (NP) license and DEA (or eligibility)
- Experience in geriatrics, chronic disease management, wound care, injections, ABIs, or imaging interpretation is preferred
- Proficiency with electronic medical records (EMR), Athena preferred
- Bilingual Spanish-speaking ability is highly valued
