Nurse Practitioner (NP) – Summary
- Deliver value-based primary care to Medicare patient populations.
- Work as part of a multidisciplinary team, including physicians, scribes, medical assistants, and social workers.
- Utilize on-site laboratory services and specialist consultation platforms.

Duties & Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans based on evidence-based guidelines.
- Manage a patient panel with an average daily volume of 13-16 patients.
- Perform regular patient visits (every three months on average, with more frequent visits for complex cases).
- Allocate sufficient time for each visit (20 minutes for routine visits, 40 minutes for diagnostics and new patients).
- Coordinate care with multidisciplinary team members, including scribes, medical assistants, and social workers.
- Participate in preventive health activities and community wellness programs such as exercise classes, yoga, and educational events.
- Utilize electronic medical record systems (Canopy and Greenway) to document patient interactions and outcomes accurately.
- Employ value-based healthcare principles to optimize care delivery and reduce unnecessary hospitalizations.
- Engage in ongoing professional development and continuing medical education.
- Adhere to all clinical protocols, compliance standards, and regulatory requirements.

Qualifications & Requirements
- Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner certificate program.
- Active, non-probationary state Nurse Practitioner license.
- Current DEA license for prescriptive authority.
- National certification in Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
- Experience or familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments preferred.
