We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our healthcare team and provide high-quality, value-based care for Medicare patient populations in Portsmouth, VA. Nurse Practitioner (NP) - Summary

Duties & Responsibilities
- Conduct patient assessments, routine visits (approximately every 3 months), and comprehensive diagnostic evaluations
- Manage an average daily patient volume of 13-16, with regular and full diagnostic visits ranging from 20 to 40 minutes
- Provide preventive, acute, and chronic disease management
- Collaborate closely with care team members, including physicians, scribes, medical assistants, and social workers
- Document patient care accurately using Canopy and Greenway EMR systems
- Utilize on-site labs and coordinate specialist referrals through RubiconMD as needed
- Participate in patient engagement programs, including community room activities such as exercise sessions, wellness events, and educational seminars
- Support value-based care initiatives and contribute to reducing hospital admissions
- Ensure compliance with all regulatory and documentation standards, including HCC, ICD-10, and HRAs (preferred)

Qualifications & Requirements
- Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner certificate program
- Current national certification in at least one of the following: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
- Active, unrestricted state Nurse Practitioner license
- Active DEA license
- Knowledge of HCC, ICD-10 coding, and Health Risk Assessments is preferred
