The Nurse Practitioner role involves managing a panel of Medicare patients, providing regular follow-up visits approximately every three months, and more frequent visits for patients with complex health needs. Patient visits are structured to allow for 20-minute regular appointments and 40-minute sessions for new or diagnostic evaluations. The average daily patient volume ranges from 13 to 16, ensuring you can deliver personalized care to each individual. You will also participate in community room activities that promote patient wellness, such as exercise classes, yoga sessions, and health-related events, fostering a holistic approach to patient engagement and preventive care.

Nurse Practitioner (NP) – Summary- Full-time Nurse Practitioner (NP) position focused on Medicare patient populations

- National certification as a Nurse Practitioner in at least one of the following: Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), Adult Nurse Practitioner (ANP), or Gerontological Nurse Practitioner (GNP)
- Active, non-probationary Nurse Practitioner license
- Active DEA license
- Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments (preferred)
- Bilingual Spanish-speaking candidates encouraged to apply
