Nurse Practitioner (NP) - Summary
- Deliver value-based primary care to Medicare patients in a collaborative, team-based setting.
- Prioritize preventive medicine and patient outcomes over patient volume.
- Work closely with a multidisciplinary care team, including physicians, scribes, medical assistants, and social workers.
- Utilize advanced EMR systems and access on-site lab services and specialist consultations.
Duties & Responsibilities
- Provide comprehensive patient care, managing an average daily patient volume of 13-16 individuals.
- Conduct regular visits (20 minutes per patient) and thorough diagnostic or new patient visits (40 minutes each).
- Develop and implement personalized care plans and preventive strategies for each patient.
- Perform health risk assessments, manage chronic conditions, and ensure proper documentation (including HCC and ICD-10 coding, if experienced).
- Collaborate with care teams to optimize patient outcomes and deliver coordinated care.
- Engage patients through community activities such as exercise sessions, events, and wellness programs.
- Utilize on-site resources, including laboratory and phlebotomy services, and leverage teleconsultations via RubiconMD for specialist input.

Qualifications & Requirements
- Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner certificate program.
- Current national certification in one or more of the following specialties: Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), Adult Nurse Practitioner (ANP), or Gerontological Nurse Practitioner (GNP).
- Active, non-probationary state Nurse Practitioner license.
- Current DEA license.
- Familiarity with HCC documentation, ICD-10 coding, and health risk assessments preferred.
