Nurse Practitioner (NP) - Summary- Full-time Nurse Practitioner (NP) role specializing in comprehensive adult and geriatric care

- Identify and address care gaps by collaborating with primary care providers
- Manage and evaluate non-life-threatening acute medical conditions
- Provide support in chronic disease management, preventive care, and advance care planning
- Document assessments and interventions accurately in the electronic health record system

- Active and unrestricted Nurse Practitioner (NP) license in Oregon
- ANCC or AANP certification required
- 3-5 years of experience as a Nurse Practitioner (NP), preferably in Primary Care, Geriatrics, Internal Medicine, Family Medicine, or Palliative Care
- DEA licensure or eligibility to obtain
- CPR/BLS certification required
- Spanish language proficiency is a plus, but not required
- Demonstrates compassion, empathy, and commitment to patient-centered care
- Experienced in managing complex adult and geriatric patients
- Strong interpersonal and collaborative skills for working effectively with an interdisciplinary team
- Proficient in electronic health record systems
- Flexible and adaptable to providing care in various settings, including home visits and telehealth
