Nurse Practitioner (NP) - Summary- Full-time Nurse Practitioner (NP) role in comprehensive adult and geriatric care
- Focus on outcomes, quality, and value-based care delivery

- Opportunity to serve a diverse patient population in Salem, OR
- Conduct comprehensive assessments for adults and geriatric patients in various healthcare settings, including clinics, patient homes, and through telehealth
- Manage transitions of care to minimize avoidable hospitalizations and emergency department visits
- Identify clinical care gaps and work collaboratively with primary care providers to optimize and implement individualized care plans
- Evaluate and manage non-life-threatening, acute medical conditions
- Support the management of chronic diseases, provide preventive care, and facilitate advance care planning discussions
- Competitive salary up to $150,000 annually
- $10,000 sign-on bonus for qualified Nurse Practitioner (NP) hires

- Current Nurse Practitioner (NP) license valid in Oregon
- Certification from ANCC or AANP (American Nurses Credentialing Center or American Association of Nurse Practitioners)
- Three to five years of experience as an NP in primary care, geriatrics, internal medicine, family medicine, or palliative care (preferred)
- DEA licensure or eligibility to obtain
- Active CPR/BLS certification
- Spanish bilingual skills considered a plus
- Demonstrates compassion, strong clinical judgment, and a commitment to patient-centered care
- Excels in collaborative, interdisciplinary team settings
- Experienced in adult and geriatric healthcare, including chronic disease management and preventive care
- Adaptable to various care environments and comfortable with telehealth platforms
