Nurse Practitioner (NP) – Summary

Duties & Responsibilities
- Conduct comprehensive health assessments and preventive care visits in patients’ homes, following established clinical guidelines.
- Develop and implement evidence-based treatment plans for managing acute and chronic conditions.
- Collaborate with physicians and interdisciplinary care teams to deliver coordinated, holistic care.
- Order, interpret, and evaluate diagnostic tests to inform clinical decision-making.
- Prescribe medications and therapies within the scope of licensure and applicable regulations.
- Provide patient education to empower individuals and families in the proactive management of health conditions.
- Maintain accurate and timely clinical documentation in compliance with regulatory requirements.
- Participate in ongoing quality improvement initiatives to enhance patient outcomes and healthcare delivery.

Qualifications & Requirements
- Master’s degree in Nursing (MSN) from an accredited institution required.
- Current, unrestricted RN and NP licenses in California.
- Minimum of one year of experience as a Nurse Practitioner (NP) in a clinical setting.
- DEA license preferred.
- Demonstrated commitment to patient-centered, value-based healthcare.
- Strong clinical assessment, critical thinking, and communication skills.
- Ability to work independently and collaboratively in a multidisciplinary team environment.
