Nurse Practitioner (NP) - Summary- Full-time Nurse Practitioner role in a collaborative, mission-driven primary care setting
- Based at a modern outpatient clinic serving a diverse patient population
- Work alongside a supportive healthcare team with a major regional health plan partnership
- Provide comprehensive primary care to patients of all ages, from newborns through geriatrics
- Conduct physical exams, diagnose, and treat common acute and chronic conditions
- Perform health screenings, order and interpret diagnostic tests
- Develop and implement patient management plans, including follow-ups and referrals
- Manage transitions of care and coordinate with multidisciplinary team members
- Maintain accurate, up-to-date patient records using electronic health systems
- Educate patients and families on health maintenance, disease prevention, and wellness strategies
- Utilize bilingual (Spanish/English) skills to enhance communication and care delivery, if applicable

- Master’s degree in Nursing or equivalent advanced practice nursing education
- Board certification as a Nurse Practitioner
- Experience in primary care or family medicine preferred
