Nurse Practitioner (NP) - Summary
- Deliver primary care to a predominantly older adult patient panel in a value-based care model.
- Work as part of an interdisciplinary team, including medical assistants, schedulers, referral specialists, social workers, and pharmacists.
- Support patient care in various settings, including the clinic, patient homes, and through telehealth solutions.
- Ensure high-quality, comprehensive, and coordinated care focused on achieving positive health outcomes and patient satisfaction.
- Participate in a weekday work schedule, contributing to an excellent work-life balance.
Duties & Responsibilities
- Conduct comprehensive patient assessments, physical examinations, and diagnostic evaluations.
- Manage chronic illnesses such as diabetes, COPD, heart failure, dementia, and general debility.
- Develop, implement, and monitor individualized treatment plans based on each patient’s unique needs and health goals.
- Educate patients and their families on medications, nutrition, disease progression, and health maintenance strategies.
- Collaborate closely with interdisciplinary team members to ensure coordinated, high-quality care delivery.
- Prescribe medications and order appropriate interventions within the state-defined NP scope of practice.
- Document all patient interactions, evaluations, and treatment plans thoroughly and in accordance with risk adjustment and compliance standards using EMR systems (Athena experience preferred).
- Provide care in the clinic and, as needed, in patient homes or via telehealth, adapting to the unique needs of an older adult population.
- Participate in a shared call schedule with support from a call center, ensuring minimal after-hours disruptions.

Qualifications & Requirements
- Master’s degree in Nursing from an accredited institution.
- Current board certification as a Nurse Practitioner (Adult NP, Adult-Gero NP, or Family NP).
- At least 2 years of experience in outpatient primary care as a Nurse Practitioner.
- Demonstrated ability to practice autonomously and manage complex chronic diseases.
- Experience with geriatrics, chronic disease management, wound care, injections, ABI, or imaging interpretation preferred.
- Previous experience in value-based care models is highly desirable.
- Active Nurse Practitioner license in the state of Indiana (or eligibility for licensure).
- DEA registration (or eligibility).
- Proficiency with electronic medical records; Athena EMR experience preferred.
- New graduates will not be considered for this position.
