Nurse Practitioner (NP) – Summary

Duties & Responsibilities
- Deliver comprehensive primary care to a panel of approximately 650 patients, primarily seniors with complex comorbidities such as heart disease, diabetes, and hypertension.
- Provide high-quality, value-based care in an outpatient clinic setting, focusing on continuity and patient satisfaction.
- Average 12–14 patient visits per day, enabling thorough, personalized care.
- Perform office-based procedures including suturing, joint injections, skin biopsies, and basic wound care as needed.
- Collaborate with your dedicated care team, including medical assistants, schedulers, and referral specialists, to ensure seamless, coordinated care for all patients.
- Participate in a fair call schedule rotation, based on the total number of providers in the market.
- Engage patients in wellness activities and help connect them with clinic resources.
- Contribute to achieving organizational goals in patient satisfaction, HEDIS compliance, and reduction of hospital readmissions.
- Demonstrate and uphold the organization’s values in all interactions with patients, families, and colleagues.
Salary & Benefits
- Competitive base salary with total earning potential exceeding $143,000, including up to a 10% performance-based bonus.

Qualifications & Requirements
- Active, unrestricted Nurse Practitioner (NP) license.
- At least 2 years of experience in primary care outpatient settings.
- Experience with geriatric populations is a plus but not required.
- Current DEA registration required.
- Proficiency in office-based procedures including suturing, joint injections, skin biopsies, and wound care.
- Strong computer and electronic health record (EHR) skills.
- Excellent communication, teamwork, and organizational abilities.
