Nurse Practitioner (NP) – Summary- Full-time, outpatient primary care role serving seniors in Fort Lauderdale, FL
- Physician-led, mission-driven organization focused on value-based care
- Small patient panel for personalized attention and continuity of care
- Provide comprehensive primary care services to an assigned panel of senior patients, averaging 18 patients per day
- Manage chronic conditions (typically 3-5 per patient), coordinate care, and develop individualized treatment plans
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, and referral specialist
- Utilize value-based care strategies to promote improved health outcomes
- Participate in clinic hours Monday through Friday, with administrative time included in the schedule
- Engage in holistic health initiatives and collaborate with in-house specialists in cardiology, podiatry, and other fields
- Ensure thorough documentation and compliance with clinical protocols and best practices
- Deliver culturally competent care, supporting a bilingual (Spanish-speaking) patient base
