Nurse Practitioner (NP) – Summary- Full-time, 100% outpatient primary care role

- Provide comprehensive primary care for adult and senior patients

- Coordinate with an integrated care team, including physicians, medical assistants, and other providers
- Administer health assessments, screenings, and tests
- Perform comprehensive and risk assessments, with attention to accurate clinical documentation and coding

- Current California Nurse Practitioner license
- Board Certified NP in good standing (ANCC/AANP certification required)
- Minimum of 2 years clinical experience as an NP in primary care, family medicine, internal medicine, gerontology, or palliative care
- Working knowledge of chronic care management and medication options
- Current DEA licensure and prescriptive authority (post-hire)
- Active CPR/BLS certification
- Experience with HCC coding strongly preferred
- Valid California driver’s license
