Nurse Practitioner (NP) – Summary
- Deliver comprehensive, patient-focused care in a value-based clinical model.
- Work closely with a multidisciplinary team including physicians, scribes, medical assistants, and social workers.
- Utilize advanced EMR systems and on-site laboratory resources.

Duties & Responsibilities
- Provide primary care services and preventive medicine to Medicare patient populations.
- Diagnose and manage acute and chronic conditions, focusing on value-based outcomes.
- Conduct patient visits averaging 13-16 patients per day, with 20-minute standard appointments and 40-minute diagnostic/new patient sessions.
- Perform regular follow-up visits, adjusting frequency based on patient complexity.
- Document care using Canopy and Greenway EMR systems, ensuring compliance with HCC, ICD-10, and HRA guidelines.
- Collaborate with clinic staff to coordinate care, manage transitions, and ensure quality patient experiences.
- Participate in community wellness initiatives and patient engagement activities.
- Access on-site lab services and specialist consultations through RubiconMD.
- Support patients with transportation needs and promote participation in health-related community events.

Qualifications & Requirements
- Graduate of an accredited Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner certificate program.
- National certification in at least one of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
- Active, unrestricted Nurse Practitioner license and current DEA license.
- Proficiency in Spanish is strongly preferred to serve our diverse patient population.
- Experience or familiarity with value-based care, HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs) is preferred.
