Nurse Practitioner (NP) - Summary
- Full-time advanced practice provider position serving Medicare patient populations

Duties & Responsibilities
- Provide comprehensive primary care to a panel of Medicare patients, emphasizing preventive medicine and chronic disease management
- Conduct patient visits averaging 13-16 per day, with regular visits lasting approximately 20 minutes and full diagnostic or new patient visits lasting up to 40 minutes
- Perform routine follow-ups every three months or more frequently for complex cases
- Work closely with a care team including a scribe, medical assistant, and social worker to ensure high-quality, coordinated care
- Utilize on-site laboratory services and access specialist consults via RubiconMD to streamline patient management
- Document clinical encounters accurately using Canopy and Greenway electronic medical record (EMR) systems
- Participate in community-based initiatives including wellness activities, exercise, patient events, and health education programs
- Promote value-based healthcare by reducing unnecessary hospitalizations and optimizing patient outcomes

Qualifications & Requirements
- Graduation from a Master of Science in Nursing, Doctor of Nursing Practice, or accredited Nurse Practitioner Certificate Program
- Active national certification in Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), Adult Nurse Practitioner, or Gerontological Nurse Practitioner
- Current, unrestricted Nurse Practitioner license in the state of practice
- Valid and active DEA license
- Bilingual English/Spanish proficiency preferred
- Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments preferred
