Our Nurse Practitioner (NP) positions are structured to reward your dedication and outcomes through competitive compensation and incentive bonuses. The full-time schedule provides stability and predictable hours, with most days running Monday through Friday. You will typically see 13-16 patients per day, allowing time for both thorough regular visits and comprehensive diagnostic or new patient appointments. Nurse Practitioner (NP) - Summary- Deliver value-based, quality healthcare to Medicare patient populations in Chicago, IL
- Collaborate with a multidisciplinary care team to ensure comprehensive patient support
- Focus on preventive medicine and chronic disease management

- Perform routine and diagnostic visits, averaging 13-16 patients per day, with visit times tailored to patient needs
- Utilize EMR systems such as Canopy and Greenway for efficient clinical documentation
- Collaborate with care team members, including scribes, MAs, and social workers
- Order and interpret labs, coordinate with specialists using platforms like RubiconMD
- Participate in preventive health initiatives and community wellness activities
- Ensure compliance with value-based care protocols and organizational guidelines
- Document and code visits accurately using HCC and ICD-10 standards (preferred)

- Graduate of an accredited Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program
- National board certification as a Nurse Practitioner in one of the following specialties: Family (FNP), Adult-Gerontology Primary Care (AGPCNP), Adult (ANP), or Gerontological (GNP)
- Active, non-probationary state Nurse Practitioner license
- Current DEA license
- Experience with HCC documentation, ICD-10 coding, and Health Risk Assessments preferred
- Demonstrates commitment to patient-centered, value-based care
- Strong clinical assessment, communication, and documentation skills
- Collaborative approach, thriving in multidisciplinary team environments
- Experience with Medicare populations and chronic disease management
