Nurse Practitioner (NP) – Summary

Duties & Responsibilities
- Deliver comprehensive primary care to Medicare patients, focusing on prevention, chronic disease management, and patient education
- Conduct regular patient visits (average 13-16 patients per day; 20 minutes for regular visits, 40 minutes for new or complex cases)
- Collaborate with a care team including scribes, medical assistants, social workers, and physicians
- Utilize on-site laboratory services and EMR systems (Canopy, Greenway) for efficient patient management
- Coordinate and participate in community-based wellness activities for patients
- Document patient encounters and care plans accurately, including HCC documentation and ICD-10 coding when applicable
- Ensure compliance with regulatory and clinical guidelines
- Engage in ongoing education and training to stay current with best practices

Qualifications & Requirements
- Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner certificate program graduate
- Active national certification as a Family Nurse Practitioner, Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP
- Current, non-probationary state Nurse Practitioner license
- Active DEA license
- Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments preferred
