Nurse Practitioner (NP) - Summary

Duties & Responsibilities
- Deliver direct patient care in an outpatient clinical setting, managing acute and chronic conditions
- Conduct comprehensive patient assessments, formulate diagnoses, and develop individualized treatment plans
- Utilize onsite resources, such as laboratory services and specialist access, to enhance patient care
- Maintain accurate and thorough documentation using EMR systems (Canopy and Greenway)
- Collaborate with multidisciplinary care teams, including physicians, scribes, medical assistants, and social workers
- Monitor patient progress, adjust care plans as needed, and coordinate follow-up care
- Educate patients and their families on health management, disease prevention, and wellness strategies
- Participate in value-based care initiatives designed to reduce hospital admissions and improve outcomes
- Engage in community activities and patient events to promote wellness and build relationships
- Manage a patient panel with an average daily volume of 13-16 patients, offering regular and diagnostic visits
- Utilize HCC documentation, ICD-10 coding, and Health Risk Assessments as applicable

Qualifications & Requirements
- Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program
- National certification as a Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
- Active, unrestricted Nurse Practitioner license
- Active DEA license
- Knowledge of HCC documentation, ICD-10 coding, and Health Risk Assessments is preferred
