Nurse Practitioner (NP) - Summary

Duties & Responsibilities
- Provide direct patient care, including assessments, diagnosis, and treatment planning
- Manage a daily patient volume averaging 13-16 individuals
- Conduct regular and comprehensive patient visits (20-minute standard, 40-minute for diagnostics/new patients)
- Follow up with patients on a consistent schedule, with increased frequency for complex cases
- Collaborate closely with physicians, medical assistants, scribes, and social workers
- Utilize onsite laboratory and diagnostic resources
- Leverage EMR systems (Canopy and Greenway) for accurate documentation and care coordination
- Participate in value-based care initiatives to minimize hospital admissions and promote preventive care
- Engage patients in community room activities, such as exercise, wellness events, and educational programs
- Support patients with access to transportation and specialist consultations as needed

Qualifications & Requirements
- Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or NP Certificate Program graduate
- National certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), Adult Nurse Practitioner (ANP), or Gerontological Nurse Practitioner (GNP)
- Active, non-probationary state Nurse Practitioner license
- Active DEA license required
- Experience or familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments (preferred)
