In this Nurse Practitioner (NP) role, you will manage a panel of Medicare patients, providing comprehensive primary care and engaging with patients every three months or more frequently as complexity requires. The position emphasizes a value-based care approach, rewarding outcomes and preventive interventions through competitive compensation and bonus structures. You will be equipped with robust clinical resources, including on-site laboratory services, access to a scribe, medical assistant, social worker, and the ability to consult with specialists through RubiconMD. Nurse Practitioner (NP) – Summary- Full-time position focused on Medicare patient populations
- Deliver value-based healthcare with a strong support system

- Provide comprehensive primary care to Medicare patients, with an average of 13-16 patient visits per day
- Conduct regular visits (20 minutes each), full diagnostic and new patient visits (40 minutes each)
- Manage ongoing patient care, seeing patients every three months or more frequently as needed
- Work alongside a team including a scribe, medical assistant, and social worker
- Utilize EMRs such as Canopy and Greenway for documentation and care coordination
- Access on-site laboratory services and specialist consultations through RubiconMD
- Participate in value-based healthcare initiatives to reduce hospital admissions and improve preventive care
- Engage patients in wellness activities and community room programs
- Support transportation and patient engagement initiatives to improve access to care

- National certification in one or more of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, Gerontological Nurse Practitioner
- Active, non-probationary Nurse Practitioner license
- Active DEA license
- Proficiency with HCC documentation, ICD-10 coding, and Health Risk Assessments is preferred
- Bilingual in Spanish preferred
