Nurse Practitioner (NP) - Summary
- Deliver primary care services to Medicare patients within a value-based healthcare model.
- Work collaboratively with a multidisciplinary care team, including physicians, scribes, medical assistants, and social workers.
- Utilize industry-leading tools and resources, such as on-site labs and access to medical specialists via RubiconMD, to enhance patient care.
- Leverage electronic medical records (EMR) systems—Canopy and Greenway—to maintain accurate and efficient documentation.
- Engage patients through comprehensive visits, including routine, diagnostic, and new patient appointments with flexible scheduling based on complexity of care.
Duties & Responsibilities
- Conduct thorough patient assessments, develop care plans, and manage ongoing primary care for a panel of 13-16 patients per day.
- Perform 20-minute regular visits and 40-minute diagnostic or new patient visits.
- Evaluate and manage chronic and acute medical conditions, emphasizing preventive medicine and health education.
- Collaborate closely with an interdisciplinary team to coordinate holistic patient care and reduce hospital admissions.
- Participate in community room activities, such as exercise classes and wellness events, fostering a supportive environment for patients.
- Ensure proper documentation of medical records in accordance with HCC, ICD-10, and Health Risk Assessment protocols (preferred).
- Utilize on-site laboratory services and coordinate with other healthcare professionals to deliver comprehensive care.
- Demonstrate commitment to value-based healthcare, prioritizing quality outcomes and patient satisfaction.

Qualifications & Requirements
- Graduation from a Master of Science in Nursing, Doctor of Nursing Practice, or accredited Nurse Practitioner certificate program.
- Current national certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
- Active, non-probationary Nurse Practitioner license in good standing.
- Current DEA license required.
- Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments preferred.
