Nurse Practitioner (NP) - Summary

Duties & Responsibilities
- Conduct regular patient evaluations, health risk assessments, and develop patient-centered care plans.
- See an average of 13-16 patients per day, with visit lengths tailored to patient needs (20 minutes for routine visits, 40 minutes for new or complex cases).
- Coordinate care with support staff, including scribes, medical assistants, and social workers.
- Utilize on-site lab resources and collaborate with specialists via RubiconMD as needed.
- Maintain accurate documentation using EMR systems such as Canopy and Greenway.
- Participate in value-based care initiatives to reduce hospital admissions and enhance preventive health measures.
- Engage patients in community room activities and support their overall well-being.
- Contribute to a positive, inclusive, and collaborative workplace culture.

Qualifications & Requirements
- Graduation from an accredited Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program.
- Current national certification in Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
- Active, non-probationary Nurse Practitioner license in the state of practice.
- Active DEA license required.
- Experience or familiarity with HCC documentation, ICD-10 coding, and health risk assessments preferred.
