We are seeking a dedicated and motivated Nurse Practitioner (NP) to join our collaborative care team in Akron, OH. The clinic environment fosters collaboration among a diverse group of healthcare professionals, including other Nurse Practitioners (NP) and Primary Care Physicians, typically with 3-5 providers per clinic.
As a Nurse Practitioner (NP), your daily patient volume will be manageable, allowing you to provide thorough care. You will see an average of 13-16 patients per day, with visit durations tailored to patient needs—20 minutes for standard appointments and 40 minutes for full diagnostic or new patient visits. Patients are generally seen every three months, with increased frequency for complex cases. Our value-based care model emphasizes preventive care, patient engagement, and the reduction of unnecessary hospitalizations, aligning with our mission to deliver the highest standard of care to the Medicare population.

Nurse Practitioner (NP) - Summary- Advanced practice provider role focused on value-based care for Medicare patients
- Full-time position in Akron, OH
- Collaborative care team environment with on-site support staff

- Conduct routine and diagnostic patient visits (13-16 patients per day)
- Collaborate with care team including scribe, MA, and social worker
- Utilize on-site laboratory and phlebotomy services for diagnostics
- Coordinate care and consult with specialists using RubiconMD
- Document all care accurately using Canopy and Greenway EMR systems
- Participate in patient engagement activities and community events
- Emphasize preventive care and chronic disease management
- Support reduction of avoidable hospital admissions through effective care planning
- Competitive salary range: $105,000–$140,000 base, with incentive bonuses up to $8,000

- Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of a Nurse Practitioner certificate program
- Current national certification as a Nurse Practitioner (NP) in Family, Adult-Gerontology Primary Care, Adult, or Gerontological specialty
- Active, non-probationary state Nurse Practitioner license
- Active DEA license required
- Preferred: Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs)
