Nurse Practitioner (NP) – Summary
- Full-time advanced practice provider role focused on value-based care
- Deliver primary care to Medicare patient populations in Chicago, IL
- Collaborate with a multidisciplinary team to ensure optimal patient outcomes
Duties & Responsibilities
- Provide comprehensive primary care to a panel of Medicare patients, focusing on preventive medicine and chronic disease management
- Conduct patient visits, including regular check-ups, new patient assessments, and full diagnostic evaluations
- Maintain an average daily patient volume of 13-16 patients, allowing for meaningful patient interactions
- Utilize electronic medical record (EMR) systems such as Canopy and Greenway for accurate documentation
- Collaborate with a care team that includes scribes, medical assistants, and social workers
- Participate in community health initiatives and patient engagement activities offered at the clinic
- Support value-based care initiatives by focusing on outcome-driven care and minimizing unnecessary hospital admissions
- Engage in ongoing professional development and contribute to a positive team environment
- Complete Health Risk Assessments (HRAs) and utilize HCC documentation and ICD-10 coding as needed
- Ensure compliance with all regulatory and quality assurance standards

Qualifications & Requirements
- Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or graduate of a Nurse Practitioner certificate program
- National certification in Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
- Active, unrestricted Nurse Practitioner license
- Active DEA license
- Familiarity with HCC, ICD-10 coding, and HRAs preferred
- Demonstrated commitment to value-based and patient-centered care
