Nurse Practitioner (NP) - Summary- Role: Nurse Practitioner (NP) in a primary care practice
- Patient Population: Adult and geriatric, ages 18+
- Team-based, collaborative care model
- Focus on health promotion, prevention, and chronic disease management
- Conduct comprehensive health assessments, including physical examinations and patient histories
- Diagnose and manage acute and chronic medical conditions for adult and senior patients
- Develop and implement individualized, evidence-based treatment plans
- Provide education and counseling to patients and families on disease prevention and healthy lifestyle choices
- Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care
- Document all patient interactions and care plans accurately in the electronic health record
- Participate in quality improvement initiatives within the practice
