Nurse Practitioner (NP) - Summary
- Position: Full-time Nurse Practitioner (NP)
- Setting: Outpatient Primary Care Clinic
- Location: Omaha, NE
- Patient Focus: Seniors with multiple chronic conditions
- Clinic Hours: Monday to Friday, 8:00 a.m. – 5:00 p.m.
Duties & Responsibilities
- Deliver comprehensive primary care to a panel of up to 650 senior patients, typically managing 10-12 appointments per day.
- Utilize a value-based care approach to manage chronic conditions such as heart disease, diabetes, and hypertension.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, referral specialist, and supportive leadership.
- Perform office-based procedures including suturing, joint injections, skin biopsies, and basic wound care.
- Provide compassionate, patient-centered care and serve as a trusted healthcare partner to patients and their families.
- Participate in a rotating call schedule, proportionate to the number of providers in the practice.
- Promote wellness through clinic-based activities and community health programs.
- Document all patient encounters thoroughly and maintain compliance with clinic policies and regulations.
- Engage in continuous performance improvement and uphold high standards for HEDIS compliance, patient satisfaction, and organizational values.

- Competitive base salary with a total earning potential exceeding $143,000, including up to 10% performance-based bonus.

Qualifications & Requirements
- Active and unrestricted Nurse Practitioner (NP) license.
- At least 2 years of experience in outpatient primary care; geriatrics experience preferred but not required.
- Current DEA registration.
- Demonstrated proficiency in office procedures, including suturing, joint injections, skin biopsies, and wound care.
- Excellent interpersonal and communication skills with a commitment to patient-centered care.
- Proficiency with electronic health records and basic office technology.
- Ability to work collaboratively within a multidisciplinary team environment.
