Nurse Practitioner (NP) - Summary- Full-time, weekday position in an outpatient clinical setting
- No on-call duties—standard Monday through Friday schedule (9am-5pm)
- Focuses on management and education of patients with chronic conditions
- Collaborative, multidisciplinary team environment
- Provide comprehensive management for patients with chronic health conditions
- Monitor patient progress and proactively adjust treatment plans as clinically indicated
