Nurse Practitioner (NP) - Summary
- Full-time, weekday position in an outpatient clinical setting

Duties & Responsibilities
- Provide comprehensive management for patients with chronic health conditions
- Monitor patient progress and proactively adjust treatment plans as clinically indicated

- Document all patient interactions and care plans accurately in the medical record system
- Maintain strict adherence to established clinical standards of care and professional best practices
- Collaborate with physicians, nurses, and allied health professionals to ensure coordinated patient care

Qualifications & Requirements
- Current and unrestricted Nurse Practitioner (NP) license
- Graduate degree from an accredited Nurse Practitioner program
- Board certification as a Nurse Practitioner (NP)
- Proven experience in managing chronic conditions in an outpatient setting preferred
