Nurse Practitioner (NP) – Summary- Full-time role providing equitable, value-based healthcare services to Medicare patient populations in Chicago.
- Collaborative, multidisciplinary care environment with a focus on preventive medicine and patient outcomes.
- Supported by large care teams to ensure quality care delivery and excellent patient experiences.
- Deliver comprehensive primary care to patients, including regular and diagnostic visits.
- Work closely with care teams, including scribes, medical assistants, and social workers.
- Utilize EMR systems (Canopy and Greenway) for efficient patient documentation.
- Participate in value-based care initiatives to improve patient outcomes and satisfaction.
- Engage in patient education, preventive health activities, and community health programs.
- Contribute to reduced hospital admissions and support chronic disease management.
- Provide care coordination and work with specialists via RubiconMD as needed.
- Support and participate in on-site labs, diagnostic testing, and patient wellness events.

- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
- National certification in at least one specialty: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
- Active, non-probationary state Nurse Practitioner license.
- Active DEA license required.
- Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments preferred.
- Bilingual Spanish-speaking skills preferred.
- Experienced Nurse Practitioner (NP) committed to delivering value-based, patient-centered care.
- Strong communicator with a collaborative approach to multidisciplinary teamwork.
