Nurse Practitioner (NP) - Summary- Full-time Nurse Practitioner (NP) position in a dynamic outpatient primary care setting
- Opportunity to provide care to senior patients with complex health conditions
- Supportive team environment with a focus on value-based care and patient engagement

- Deliver comprehensive primary care services to an outpatient senior population
- Manage a patient panel with an average of 10-12 patients per day, maximum panel size of 650
- Diagnose and treat chronic and acute health conditions such as heart disease, diabetes, and hypertension
- Collaborate with a care team, including medical assistants, schedulers, and referral specialists
- Utilize value-based care models to drive improved patient outcomes
- Perform office procedures including suturing, joint injections, skin biopsies, and basic wound care
- Document care accurately and maintain compliance with regulatory requirements and company policies
- Participate in quality improvement initiatives and support patient satisfaction efforts
- Engage in a rotating call schedule based on the number of providers in the market

- Current, unrestricted, active Nurse Practitioner (NP) license
- Minimum of 2 years’ experience in outpatient primary care
- Experience with geriatric patients is a plus but not required
- Current DEA registration
- Proficiency in office-based procedures such as suturing, joint injections, skin biopsies, and wound care
- Strong PC and electronic health record (EHR) skills
- Commitment to delivering patient-centered, high-quality care
