At Primary Care Plus, we are seeking a dedicated and highly skilled Nurse Practitioner (NP) to join our dynamic and physician-led healthcare team in New Orleans, LA. Our practice is committed to delivering exceptional, patient-centered care through a clinically integrated network designed to support Medicare Advantage, Quality plan, and Accountable Care Organization (ACO) members. As a Nurse Practitioner (NP) with Primary Care Plus, you will have the opportunity to work in a collaborative clinical environment that prioritizes quality outcomes, coordinated care, and the overall well-being of our diverse patient population.
Primary Care Plus clinics are supported by a multidisciplinary team that includes physicians, Advanced Practice Registered Nurses (APRNs), Registered Nurse (RN) Care Managers, clinic support staff, and a dedicated Quality Improvement team. Our team-based approach ensures comprehensive patient care across both outpatient and inpatient settings. The clinic leverages advanced electronic medical records and robust data analytics to proactively identify and support patients at risk for adverse health outcomes, ensuring that care is both proactive and personalized.
Providers at Primary Care Plus are evaluated on patient outcomes rather than productivity metrics, such as RVUs. This unique approach fosters a supportive environment focused on delivering high-quality care and achieving superior health outcomes for patients. The typical patient panel size ranges from 600 to 800 individuals, and appointments are scheduled for 30 to 60 minutes, allowing ample time for thorough assessments and patient engagement.
As a Nurse Practitioner (NP), you will play a vital role in providing comprehensive healthcare services, including health promotion, disease prevention, chronic disease management, and acute illness care. You will conduct annual comprehensive wellness assessments, develop individualized care plans, and collaborate closely with patients and their families to achieve optimal health outcomes. You will also have the opportunity to build and manage your own patient panel, with the support of a dedicated physician and access to consult on complex cases as needed.

- Manage minor acute illnesses and chronic conditions within the patient population
- Conduct comprehensive wellness assessments in the clinic or occasionally in patients’ homes
- Develop, implement, and monitor individualized care and treatment plans
- Prescribe medications within scope and as delegated by protocols
- Offer patient-centered education, teaching, and counseling to patients and families
- Participate in quality improvement initiatives and health outcome monitoring
- Ensure accurate and timely coding and documentation of quality measures and diagnoses
- Engage in community outreach, health fairs, educational events, and practice-building initiatives

- Completion of an advanced practice Nurse Education Program
- National certification as a Nurse Practitioner—Adult, Geriatric, or Family NP
- Active, unencumbered RN license in Louisiana
- CPR certification
- Minimum of 2 years’ post-graduate Nurse Practitioner experience
- Proficiency with electronic medical records
- Ability to obtain prescriptive authority post-hire (if applicable)
- Experience in Gerontology, Internal Medicine, Family Medicine, or urgent care
- Background in managed care or ambulatory care environments
- Strong administrative, organizational, and communication skills
- Demonstrated ability to work independently and collaboratively in a changing environment
