Nurse Practitioner (NP) Job DescriptionWe are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our primary care team on a full-time basis. As a Nurse Practitioner, you will play a critical role in delivering high-quality healthcare to patients in a primary care setting. This position offers an excellent opportunity for both experienced Nurse Practitioners and recent graduates to contribute to patient health and well-being in a collaborative, patient-focused environment.
Key Responsibilities- Deliver comprehensive primary care services to patients, including preventive care, health assessments, and management of acute and chronic illnesses.
- Maintain accurate, thorough, and timely patient records using electronic health record (EHR) systems, ensuring compliance with all regulatory and organizational requirements.
- Utilize available healthcare tools and resources to support patients in achieving their optimal health outcomes through education, counseling, and evidence-based interventions.
- Participate actively in care coordination efforts, collaborating with interdisciplinary team members to enhance patient satisfaction and continuity of care.
- Engage in ongoing professional development and stay current with best practices in primary care and nurse practitioner roles.
- Graduate of an accredited Nurse Practitioner (NP) program with current and valid state licensure to practice as a Nurse Practitioner.
- Experience in a primary care setting is valued; however, newly graduated Nurse Practitioners who demonstrate strong clinical skills and a passion for patient care are encouraged to apply.
- Demonstrated proficiency in using electronic health record (EHR) systems to document and manage patient information.
- Outstanding interpersonal and communication abilities, enabling effective collaboration with patients, families, and healthcare team members.
