Nurse Practitioner (NP) – Full-time About the Role
We are seeking a dedicated Nurse Practitioner (NP) to join our healthcare team in Allentown, PA. In this full-time position, you will play a vital role in delivering equitable and effective value-based healthcare services to Medicare patient populations. As a Nurse Practitioner, you will work alongside a collaborative care team to ensure patients receive comprehensive, outcome-focused medical care. Key Responsibilities
- Provide high-quality, patient-centered care to a panel of Medicare patients, focusing on value-based outcomes.
- Conduct thorough patient assessments, routine and diagnostic visits, and preventive health screenings.
- Work closely with a multidisciplinary team including scribes, medical assistants, social workers, and fellow providers to coordinate and optimize patient care.
- Utilize onsite resources such as laboratory services with phlebotomy and specialist consultations via RubiconMD to enhance patient care delivery.
- Document patient encounters and care plans accurately in electronic medical record (EMR) systems such as Canopy and Greenway.
- Adhere to value-based healthcare principles, striving to reduce unnecessary hospital admissions and improve overall patient experience.
- Engage in community and patient wellness activities, supporting exercise, educational events, and preventive programs.
- Participate in ongoing quality improvement initiatives and contribute to a positive, collaborative team environment.
Qualifications
- Graduate of a Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner Certificate Program.
- National certification in one or more of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
- Active, unrestricted Nurse Practitioner license in the state of Pennsylvania.
- Active DEA license.
- Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments (preferred).
