Nurse Practitioner (NP) – Full-time Job Overview
We are seeking a highly skilled and compassionate Nurse Practitioner (NP) to join our healthcare team in Allentown, PA. As an integral member of an advanced care team focused on value-based medicine, the Nurse Practitioner will provide high-quality, patient-centered care to the local Medicare population. This full-time role is designed for clinicians who are dedicated to improving health outcomes through preventive medicine, collaborative care, and evidence-based practice. Working as part of a multidisciplinary team, the Nurse Practitioner (NP) will have the support and resources necessary to deliver exceptional patient experiences in a forward-thinking clinical environment.
Responsibilities
- Deliver primary care services to Medicare patients in a clinic setting, emphasizing preventive health and chronic disease management.
- Conduct thorough patient assessments, including regular and full diagnostic visits, ensuring comprehensive documentation in the EMR systems (Canopy and Greenway).
- Collaborate closely with physicians, scribes, medical assistants, and social workers to coordinate care and optimize patient outcomes.
- Utilize onsite laboratory and diagnostic resources, including phlebotomy, and access to specialist consultations through RubiconMD.
- Participate in the clinic’s value-based care model, focusing on improving quality metrics, reducing unnecessary hospital admissions, and enhancing the overall patient experience.
- Engage with patients in wellness and community activities offered in the clinic’s community room, contributing to holistic patient well-being.
- Maintain accurate and timely documentation for HCC (Hierarchical Condition Categories), ICD-10 coding, and Health Risk Assessments (HRAs), as appropriate.
- Work efficiently within a team of three to five providers, including Nurse Practitioners and Primary Care Physicians, to manage a daily patient volume of approximately 13-16 individuals.
Qualifications
- Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner Certificate Program.
- Current national certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), Adult Nurse Practitioner (ANP), or Gerontological Nurse Practitioner (GNP).
- Active, non-probationary Nurse Practitioner license in the state of Pennsylvania.
- Active DEA license required.
- Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments preferred.
