Nurse Practitioner (NP) – Philadelphia, PA – Full-time
We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our innovative healthcare team in Philadelphia, PA. As a Nurse Practitioner, you will play a key role in delivering high-quality, value-based outpatient primary care to adult and senior patients in a community-focused clinic environment. Responsibilities
- Deliver primary care services to an outpatient population, focusing on adult and senior patients with complex chronic conditions, such as heart disease, diabetes, and hypertension.
- Manage a patient panel of up to 650 individuals, with an average daily caseload of 10-12 patients, ensuring continuity and quality of care.
- Operate within a value-based care model, emphasizing preventative care, patient satisfaction, and health outcomes.
- Work collaboratively as part of an integrated care team, including medical assistants, a scheduler, referral specialists, and leadership support, to coordinate all aspects of patient care.
- Perform a range of office procedures, including suturing, joint injections, skin biopsies, and basic wound care, as appropriate to your scope of practice and experience.
- Participate in a rotating call schedule, with frequency determined by the number of providers in the local market.
- Utilize electronic health records and proficient PC skills to document patient encounters and coordinate care.
Qualifications
- Current, unrestricted, active license as a Nurse Practitioner (NP) in Pennsylvania.
- Minimum of 2 years of experience in outpatient primary care is required.
- Experience in geriatrics is a plus, but not required.
- Current DEA registration.
- Proficiency in office-based procedures relevant to the role, including suturing, joint injections, skin biopsies, and basic wound care.
