Nurse Practitioner (NP) - Summary
- Full-time Nurse Practitioner (NP) role in a primary care outpatient clinic
- Focus on value-based care for a senior patient population with complex chronic conditions

Duties & Responsibilities
- Provide comprehensive primary care to a panel of senior patients, typically managing 10-12 patients per day
- Deliver care using a value-based care model, focusing on improving health outcomes and reducing hospitalizations
- Manage a panel size up to 650 patients, with support from a dedicated care team, including medical assistants, schedulers, referral specialists, and leadership
- Perform office procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Participate in a flexible call schedule, shared among providers in the market
- Work Monday through Friday, 8am-5pm, in an outpatient clinic setting
- Promote patient engagement by supporting wellness activities and providing education on disease management
- Collaborate with interdisciplinary team members to coordinate comprehensive care plans

Qualifications & Requirements
- Active and unrestricted Nurse Practitioner (NP) license
- Current DEA registration
- Minimum of 2 years of primary care outpatient experience
- Experience in geriatrics is beneficial but not required
- Proficiency in office-based procedures, including suturing, joint injections, skin biopsies, and basic wound care
- Ability to work collaboratively within a multidisciplinary team
- Commitment to value-based care and quality improvement initiatives
