As a Nurse Practitioner (NP) in our primary care outpatient setting, you will be instrumental in delivering best-in-class care for a panel of senior patients, many of whom have multiple chronic conditions such as heart disease, diabetes, and hypertension. You will work closely with a dedicated care team, including a medical assistant, scheduler, referral specialist, and receive strong support from clinic leadership. Our goal is to ensure our providers have the resources and environment necessary to focus on what matters most: the patient.
You will see an average of 10-12 patients per day, allowing ample time to build relationships and provide comprehensive care. Nurse Practitioner (NP) - Summary- Full-time Nurse Practitioner (NP) role in a patient-focused, primary care outpatient clinic
- Serve a senior patient population with complex chronic conditions
- Work within a value-based care model
- Join a collaborative care team with strong leadership support
- Deliver comprehensive primary care to adult and senior patients in an outpatient clinic setting
- Manage a patient panel with a maximum of 650 members, averaging 10-12 patients per day
- Diagnose, treat, and manage patients with chronic conditions such as heart disease, diabetes, and hypertension
- Perform office-based procedures including suturing, joint injections, skin biopsies, and basic wound care
- Collaborate with a multidisciplinary care team, including medical assistants, schedulers, and referral specialists
- Maintain thorough and accurate patient records in accordance with clinical best practices
- Participate in a rotating call schedule based on provider availability
- Contribute to clinic quality improvement initiatives and promote patient satisfaction
- Base salary of $130,000, with total earning potential up to $143,000+ through bonus incentives

- Current, unrestricted, and active license to practice as a Nurse Practitioner
- Minimum of 2 years’ experience in primary care outpatient settings
- Experience with geriatric populations preferred, but not required
- Current DEA registration
- Proficiency in office-based procedures: suturing, joint injections, skin biopsies, wound care
