Nurse Practitioner (NP) Job Overview
We are seeking a dedicated Nurse Practitioner (NP) to join our innovative healthcare team in Cleveland, OH. As a Nurse Practitioner working in a primary care outpatient setting, you will be an integral part of a value-based care model designed to improve the health and well-being of patients, particularly seniors with complex medical needs. This full-time opportunity provides a collaborative environment with a strong support team, modern clinic facilities, and a patient-centered approach.
Key Responsibilities
- Deliver comprehensive primary care services to a diverse panel of patients, with a focus on seniors managing multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Manage an outpatient caseload, typically seeing 10-12 patients per day, with a maximum panel size of 650 patients.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, referral specialist, and receive full support from clinic leadership.
- Utilize a value-based care approach to maximize patient outcomes and satisfaction.
- Perform common office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Participate in a call schedule determined by the number of providers in the market, ensuring a balanced work-life schedule.
- Adhere to clinical guidelines, quality standards, and organizational values to achieve targets in areas such as HEDIS compliance and patient satisfaction.
Qualifications
- Current, unrestricted, active license to practice as a Nurse Practitioner (NP).
- At least 2 years of experience in a primary care outpatient setting.
- Experience in geriatrics is a plus, but not required.
- Current DEA registration.
- Proficiency in office-based procedures, including suturing, joint injections, skin biopsies, and basic wound care.
- Strong commitment to delivering patient-centered, compassionate care.
- Excellent communication, organizational, and teamwork skills.
