Nurse Practitioner (NP) Job Overview
We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our innovative healthcare organization on a full-time basis in Milwaukee, WI. As an NP, you will play a key role in delivering high-quality, value-based primary care to a diverse senior patient population. Key Responsibilities
- Provide comprehensive primary care services in an outpatient setting, focusing on seniors with complex health issues such as heart disease, diabetes, and hypertension.
- Utilize a value-based care model to ensure optimal patient outcomes and satisfaction.
- Manage a patient panel of up to 650 individuals, averaging 10-12 patients per day.
- Collaborate closely with a dedicated care team, including medical assistants, schedulers, referral specialists, and leadership support.
- Participate in a shared call schedule to ensure continuous patient care coverage.
- Perform office procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed.
- Promote patient engagement through coordination of wellness activities and community health programs.
- Document clinical care and maintain compliance with organizational standards and regulatory requirements.
Qualifications
- Current, unrestricted, active Nurse Practitioner (NP) license to practice medicine.
- Current DEA registration required.
- Minimum of 2 years of primary care outpatient experience; experience in geriatrics is a plus but not required.
- Proficiency in common office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Strong interpersonal and communication skills to foster positive relationships with patients and care teams.
