Nurse Practitioner (NP) - Summary
- Provide comprehensive primary care to seniors with multiple chronic conditions in an outpatient setting
- Work collaboratively as part of an integrated care team to deliver patient-centered care
- Focus on improving patient outcomes through a value-based care approach
- Ensure high levels of patient satisfaction and continuity of care
Duties & Responsibilities
- Conduct assessments, develop care plans, and manage acute and chronic illnesses for a panel of up to 650 patients
- Average of 10–12 patient visits per day, allowing for deeper patient engagement
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Coordinate care with other healthcare professionals, including specialists and ancillary providers
- Participate in a rotating call schedule based on the number of providers in the market
- Utilize electronic medical records and maintain accurate documentation of patient encounters
- Promote wellness and preventative care initiatives within the clinic
- Engage patients in activities that enhance their overall health, including fitness and creative programs
- Monitor quality metrics such as HEDIS compliance and contribute to reducing hospital admissions
Salary & Benefits
- Competitive base salary starting at $130,000, with the potential to earn over $143,000 including performance bonuses

Qualifications & Requirements
- Active Nurse Practitioner (NP) license with current, unrestricted status
- At least 2 years of experience in outpatient primary care
- Experience working with geriatric populations is a plus, but not mandatory
- Current DEA registration
- Proficiency in basic office procedures (suturing, joint injections, skin biopsies, wound care)
- Demonstrated ability to deliver compassionate, patient-centered care
- Strong communication and teamwork skills
