Nurse Practitioner (NP) – Full-time Primary Care

As a Nurse Practitioner (NP), you will work with a dedicated care and support team, including your own medical assistant and scheduler, to ensure a seamless patient experience. You will be responsible for managing a small patient panel, allowing you to focus on relationship-building and quality outcomes. The patient population consists primarily of seniors living with 3-5 chronic diseases, and you will play a key role in managing and improving their health through a value-based care approach. Our organization offers a strong work/life balance, with no nights or weekends required and administrative time included in your schedule.
Clinic hours are structured to promote provider well-being and optimal patient care. The last patient appointment is scheduled in the late afternoon, allowing for predictable and manageable workdays.
