Nurse Practitioner (NP) Position OverviewWe are seeking a dedicated Nurse Practitioner (NP) to join our primary care team on a full-time basis. This role is an excellent opportunity for a compassionate and motivated clinician who is passionate about providing exceptional care to senior patients. As a Nurse Practitioner (NP) within our physician-led, mission-driven organization, you will have the opportunity to make a meaningful impact on the health and wellness of the community, particularly focusing on individuals with complex, chronic conditions.
Key Responsibilities- Deliver comprehensive primary care services to a panel of senior patients, with a maximum panel size of 450 individuals.
- Conduct patient assessments, develop care plans, and manage ongoing treatment for patients, primarily those with 3-5 chronic diseases.
- See approximately 18 patients per day in an outpatient setting, with dedicated administrative time included in the schedule.
- Work collaboratively with a support team, including a medical assistant and scheduler, to ensure seamless patient care and clinic operations.
- Participate in a value-based care model that prioritizes quality health outcomes and holistic well-being for seniors.
- Utilize in-house consulting specialists such as cardiology and podiatry when needed to optimize patient outcomes.
- Engage with additional holistic health services available to patients, including acupuncture, tai chi, yoga, and nutrition counseling.
- Full-time position with clinic hours from Monday to Friday, 7:30 AM to 5:00 PM; the last patient is scheduled at 4:15 PM.
- No nights or weekends are required, supporting a strong work/life balance.
- 100% outpatient care environment.
- Board certification in Family Medicine or Geriatric Medicine is required.
- Active Nurse Practitioner (NP) license in good standing.
- Demonstrated commitment to providing compassionate, patient-centered care to diverse senior populations.
- Experience working in a primary care or outpatient setting is preferred.
- Strong teamwork and communication skills for collaborating with multidisciplinary teams.
