Nurse Practitioner (NP) – Summary
- Deliver value-based healthcare to Medicare patient populations.
- Work within a collaborative care team to provide comprehensive, patient-centered care.

Duties & Responsibilities
- Conduct patient visits (13-16 per day), including regular and diagnostic appointments.
- Participate in a Monday–Friday, 8am–5pm schedule as part of a dedicated care team.
- Collaborate with scribes, medical assistants, and social workers for patient management.
- Utilize on-site labs and electronic medical records (EMR) such as Canopy and Greenway.
- Engage in care model activities including exercise, wellness events, and patient transportation support.
- Document clinical care, utilize HCC coding, ICD-10, and complete Health Risk Assessments as applicable.
- Maintain compliance with all regulatory and clinical quality standards.
- Contribute to a positive patient experience through effective communication and care coordination.

Qualifications & Requirements
- Master of Science in Nursing, Doctor of Nursing Practice, or completion of a Nurse Practitioner certificate program.
- National certification as a Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
- Active, valid Nurse Practitioner license in good standing.
- Current DEA license required.
- Spanish language proficiency preferred.
- Familiarity with HCC, ICD-10 coding, and health risk assessments is preferred.
