We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our team in Tucson, AZ. This full-time position is ideal for a healthcare professional who is passionate about delivering value-based care to Medicare patient populations. As a Nurse Practitioner (NP), you will be an integral member of a collaborative care team, working to enhance patient outcomes while focusing on preventive medicine and reducing unnecessary hospital admissions. Key Responsibilities
- Deliver high-quality, value-based healthcare services to Medicare patient populations.
- Collaborate with a multidisciplinary care team that includes physicians, scribes, medical assistants, and social workers to ensure comprehensive patient care.
- Conduct patient visits in a clinic setting, typically seeing 13-16 patients per day, with 20-minute appointments for regular visits and up to 40 minutes for full diagnostic and new patient visits.
- Utilize electronic medical records (EMR) systems, including Canopy and Greenway, to document patient interactions and maintain accurate records.
- Participate in a value-based care model focused on achieving positive patient outcomes rather than volume of services.
- Access on-site laboratory services, including a dedicated phlebotomist, and consult with specialists via RubiconMD as needed.
- Engage in community-based activities and initiatives, such as exercise classes, wellness events, and patient support programs, to promote overall health and wellness.
- Support transportation initiatives for patients within a five-mile radius of the clinic to ensure access to care.
- Contribute to a patient-centered culture that prioritizes preventive care, patient education, and a superior patient experience.
Required Qualifications
- Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or graduation from a Nurse Practitioner (NP) Certificate Program.
- National certification as a Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
- Active, non-probationary Nurse Practitioner (NP) license for the state of Arizona.
- Active DEA license.
- Bilingual in Spanish is strongly preferred.
- Familiarity with HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and Health Risk Assessments (HRAs) is preferred.
Clinic Environment and Team
- Work alongside 3-5 providers per clinic, including both Nurse Practitioners and Primary Care Physicians, to foster a supportive and collaborative clinical environment.
