Nurse Practitioner (NP) Position Overview
This full-time Nurse Practitioner (NP) opportunity is focused on delivering high-quality, value-based healthcare to Medicare patient populations. As an NP, you will work collaboratively within a large care team, allowing you to prioritize comprehensive patient care and preventive medicine over volume-based metrics. This role emphasizes patient outcomes, reducing hospital admissions, and enhancing the overall patient experience through a value-based care model.
Key Responsibilities
- Provide equitable, patient-centered healthcare to Medicare populations while adhering to a value-based care model.
- Work as part of an interdisciplinary healthcare team, including scribes, medical assistants, and social workers, to coordinate patient care and optimize outcomes.
- Conduct patient visits with a typical volume of 13-16 patients per day, allocating 20 minutes for regular visits and 40 minutes for full diagnostic and new patient appointments.
- Utilize onsite laboratory services and collaborate with specialists as needed through platforms such as RubiconMD.
- Document patient interactions and care plans using electronic medical record (EMR) systems, including Canopy and Greenway.
- Participate in community room activities and wellness events that foster patient engagement and preventive health, such as exercise classes, yoga, and health-focused events.
- Contribute to a collaborative clinical environment, typically working alongside 3-5 providers, including other NPs and Primary Care Physicians.
- Support the organization’s mission to drive preventive care, reduce unnecessary hospitalizations, and ensure a positive patient experience.
Required Qualifications
- Graduation from a Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner Certificate Program.
- National certification as a Nurse Practitioner in one or more of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
- Active, non-probationary state Nurse Practitioner license.
- Active DEA license.
- Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments is preferred.
