Nurse Practitioner (NP) - Summary
- Deliver value-based primary care to Medicare patients
- Work collaboratively within a multidisciplinary care team

Duties & Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans
- Deliver direct patient care through regular and diagnostic visits
- Manage a patient panel of 13-16 patients per day with support from scribes, medical assistants, and social workers
- Utilize EMR systems (Canopy and Greenway) for accurate documentation and care coordination
- Collaborate with onsite and remote specialists via platforms such as RubiconMD
- Participate in care team meetings to discuss patient progress and optimize outcomes
- Ensure compliance with value-based care models, focusing on preventive medicine and reducing hospital admissions
- Engage with patients in community room activities, including exercise classes, wellness events, and health education
- Provide guidance and support for patients utilizing transportation and other clinic resources

Qualifications & Requirements
- Graduate of an accredited Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner (NP) certificate program
- Current national certification in one or more of the following areas: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments (preferred)
