Nurse Practitioner (NP) - Summary
- Deliver value-based primary care to Medicare patient populations in New Haven, CT
- Work collaboratively within a multidisciplinary care team
- Utilize advanced electronic medical record (EMR) systems
- Focus on preventive care and quality outcomes
Duties & Responsibilities
- Provide comprehensive healthcare services to a panel of patients, with a focus on Medicare beneficiaries
- Conduct regular and diagnostic patient visits, allocating 20 to 40 minutes per appointment based on patient needs
- Work closely with a multidisciplinary care team including scribes, medical assistants, and social workers
- Perform health risk assessments, chronic disease management, and patient education
- Utilize on-site laboratory services and collaborate with specialists via telehealth when necessary
- Document all patient interactions accurately in EMR systems (Canopy and Greenway)
- Actively participate in quality improvement initiatives and value-based care programs
- Contribute to a positive patient experience through compassionate, equitable, and culturally sensitive care
- Engage in community outreach and patient wellness activities, including exercise and educational events

Qualifications & Requirements
- Master of Science in Nursing, Doctor of Nursing Practice, or completion of a Nurse Practitioner Certificate Program
- National certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
- Active, non-probationary state Nurse Practitioner (NP) license
- Valid DEA license
- Familiarity with HCC documentation, ICD-10 coding, and health risk assessments preferred
