Nurse Practitioner (NP) Job Description
We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our healthcare team in New Haven, CT on a full-time basis. As a critical member of our value-based care model, you will focus on providing equitable, patient-centered care to the local Medicare population. This Nurse Practitioner (NP) position emphasizes quality outcomes, preventive medicine, and a collaborative approach within a supportive clinical environment, allowing you to make a significant impact on patient health and satisfaction.
Key Responsibilities
- Deliver comprehensive primary care to patients in a value-based healthcare setting, with a focus on preventive and outcome-driven care.
- Conduct medical assessments, develop treatment plans, and manage both acute and chronic conditions for a diverse Medicare patient population.
- Work collaboratively within a multidisciplinary care team, including physicians, scribes, medical assistants, and social workers, to ensure holistic patient care and optimal health outcomes.
- Utilize electronic medical records (EMR) systems such as Canopy and Greenway for accurate documentation and care coordination.
- Perform visits tailored to patient needs, including regular 20-minute appointments and 40-minute diagnostic or new patient assessments.
- Engage in patient education to promote health literacy, preventive care, and adherence to treatment plans.
- Participate in quality improvement initiatives and contribute to meeting clinic performance metrics aligned with value-based care goals.
- Coordinate with clinical support staff and leverage on-site resources such as laboratory services and specialist access via RubiconMD.
- Support patient engagement by participating in community health activities and wellness programs offered at the clinic.
Qualifications
- Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or graduation from a Nurse Practitioner (NP) Certificate Program.
- National certification as a Nurse Practitioner in at least one of the following specialties: Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), Adult Nurse Practitioner (ANP), or Gerontological Nurse Practitioner (GNP).
- Active, non-probationary state Nurse Practitioner license.
- Active DEA license for prescription authority.
- Familiarity with HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and Health Risk Assessments (HRAs) is preferred.
