The Nurse Practitioner – Nephrology (NP) position is designed for advanced practice nurses who have a passion for nephrology or internal medicine and thrive in patient-centered, outpatient settings. Nurse Practitioner – Nephrology (NP) Summary
- Provide specialized nephrology care in a primarily outpatient clinical environment
- Serve adults and geriatric populations, including dialysis and chronic kidney disease patients
- Collaborate with an experienced multidisciplinary team
- Contribute to a value-based care model focused on patient outcomes
Duties & Responsibilities
- Perform comprehensive health assessments, physical examinations, and risk evaluations for nephrology patients
- Order, interpret, and evaluate diagnostic tests to inform patient care plans
- Develop, document, and update individualized treatment plans in collaboration with physicians
- Prescribe medications, therapies, and other necessary treatments per scope of practice
- Monitor patient progress and adjust treatment plans as clinically indicated
- Educate patients and their families regarding disease management, preventive care, and lifestyle modifications
- Work with care teams including RNs, case managers, and care coordinators to ensure effective patient management
- Participate in quality improvement and value-based initiatives aimed at enhancing patient care delivery

Qualifications & Requirements
- Master's degree in Nursing (MSN) required
- Current, unrestricted RN and NP licenses in California required
- Board Certification as a Nurse Practitioner is mandatory
- Minimum of two years experience as a Nurse Practitioner, with nephrology or internal medicine exposure preferred
- Experience managing patients with chronic conditions strongly desired
- Bilingual English/Spanish skills are a plus
